How We Use Your Information
To enable us to provide you with the services, care and support you need, it is vital we hold information about you. This includes your name, address, date of birth and medical history. During the course of your care, the information we hold about you will grow.
Information will be shared on a strict need-to-know basis with staff within Prospect Hospice and with other services involved in your support to enable your care to be coordinated at all times.
There are some occasions where Prospect Hospice may be required by law to share information to protect other people. This is uncommon, and if this is the case we will always try and talk to you about this first.
Some basic details such as your name and address are also available to our fundraising and communications departments who may contact you at a later date with news about the hospice or fundraising events. If you do not want for your details to be available, please let us know.
You have a right to see the information Prospect Hospice holds on you. For more details on the information we record and how it’s used, a copy of our How We Use Your Information leaflet can be found in the related documents section at the bottom of this page.
