Swindon’s Advance Care Planning (ACP) document
An Advance Care Planning document is a resource both for the general public and health care professionals which helps individuals plan for their future care. It gives people the opportunity to think about, talk about and write down their preferences and priorities for their future care, including how they want to receive care at the end of life. It also provides opportunities for discussion with families and carers so that all may understand what is important to the patient at the end of their life. Finally it helps the individual to be clear about those things that are important to them, and document their wishes so that they can be carried out at the appropriate time.
