Supporting the day-to-day work of healthcare professionals

Prospect Hospice’s vision is excellence and choice in end-of-life care for everyone.

Achieving this is only possible through joint working with health and social care professionals across our community.

This section of our website outlines some of the ways in which we can support you in your day-to-day work caring for people approaching the end of life. Find out here about making a referral to our services, as well as our 24 hour advice line, which gives you access to information and support around the clock. See also how we can help you build on your own knowledge, skills and experience through our education and development services by clicking on the relevant links in the right-hand box.

There is a link in the Related Documents section below to the Palliative Care Handbook by Wessex Palliative Physicians (8th edition 2014). This is an interactive version, with a clickable contents page to take you straight to the correct page. To go back to the contents simply click on the link in the top right-hand corner.

Making a referral

Our Single Point of Contact (SPoC) service is the new route for referring patients to our services. Open Monday to Friday 9am to 5pm, you can make a referral in one of the following ways:

  • By calling 01793 816124
  • By faxing a referral form to 01793 815803

Referral forms are available as a Word document from the Related Documents section at the bottom this page.

At the initial point of contact, a SPoC Clinical Nurse Specialist is available for clinical discussions. At all other times, or if you need to speak to someone immediately, please call our 24 hour advice line on 01793 816109.

24 hour advice line

Around the clock support, for patients, families and healthcare professionals.

For many people living with a life-limiting illness, being able to stay at home is something that matters above almost everything else. We at Prospect Hospice want to be able to support a patient’s choice in their care, which is why we have introduced our 24 hour advice line – so that there’s always someone there to answer your questions when you seek our help.

Our advice line is available and accessible all day, every day, with calls handled by our experienced palliative care nurses, with access to an on-call doctor at all times, ready to listen to your concerns and help address any problems you’re experiencing.  Our advice line is for healthcare professionals too, and staff from care homes, to support their confidence in the care of patients with end-of-life care needs.

If you are a patient, family member or from the local healthcare community, and you need fast advice, please call 01793 816109 to access our 24 hour advice line.
