 If you, or someone you know is living with Motor Neurone Disease (MND), our experts are here to support you.

  • Prospect Hospice has teamed up with the MND Association to ensure all people living with MND with a Swindon GP have the support they need – thanks to specialists clinical professionals working with the wider community.
  • Their role is to make sure your care is well-coordinated and that you are well-supported to the end of life.
  • This advanced service offers specialist information, sign-posting, support and guidance from diagnosis and throughout your illness.

So what can we help with?

  • Our MND specialists work collaboratively with the Consultant Neurologist in the dedicated MND clinic at the Great Western Hospital (GWH) and across the community and can visit you in your own home.
  • We coordinate a bimonthly MND multidisciplinary team meeting and work closely with all members to provide a tailored individually focused service
  • As we are based at Prospect Hospice we are able to utilise all our hospice services, including the family support team, Therapy and Clinical Nurse Specialists

We can offer:

  • Information about MND, including information for you and your family to read in your own time
  • Advice and support with advance care planning
  • Referral to wider, more holistic Prospect Hospice support, including emotional support, complementary therapy and educational courses
  • Information and advice on palliative care
  • Advice, education and support for health care professionals in the hospital, community, care agencies and nursing homes
  • Referrals to community teams who can offer further specialist advice and support

  • You can also talk to one of the members of the Clinical administrative team, including Sam Bennetta who is the Administrative Team Leader, or a member of her team.

It feels as though you are part of people’s family. I value every day and will continue to give my best to the MND patients in Swindon needing our care and support”.

The Clinical Administration Team: includes Sam, Caroline, Donna, Kathy and Nicky. The team support all the clinical teams across the hospice with their admin, and for the MND team they process referrals, arrange the multi-disciplinary meetings which take place every other month – which includes attending the meeting to take in-depth notes.

They also speak to patients and their families when the clinicians are not available, chasing up their prescriptions, contacting their GP regarding any queries, booking appointments, taking messages and signposting services.

The team are far from ‘just admin’ they provide lots of specialist expert support to all our clinical teams and ensure they can concentrate on providing excellent patient care.

The MND Connect team offers information and support on all aspects of motor neurone disease (MND), including symptom control, practical management, improving quality of life, clinical research and signposting to other organisations.

The advice line is available Monday to Friday, 9am to 5pm and 7pm to 10:30pm.
